[HealthyTribes] 2020 Student Health Survey (SHS) Questionnaires drafted

Good morning,

We are excited to share this current draft of the 2020 Oregon Student Health Survey. This is a survey that was developed by the Oregon Public Health Division in partnership with the Oregon Department of Education. Please do not distribute as this is still in draft form. Final questionnaires and resources will be available on the new Student Health Survey Website soon. We look forward to sharing this resource with you as soon as it is live.

Thank you for providing feedback and expertise throughout the process to strength our system for collecting and sharing data for the NEW Student Health Survey (SHS).

As a reminder of our process, listening sessions were held among school administrators, youth, and youth-serving organizations. Additionally, feedback was gathered from local public health, behavioral health, tribes, and other community-based organizations to provide insights and share expertise. This feedback was incorporated into the current draft of the new youth survey.

If you have further feedback or questions, please contact your liaison by October 30th. While we may not be able to incorporate all feedback, we are continually assessing how we can improve this data collection tool over time. Your time and attention is truly appreciated.
