Hi Everyone -
Benton County Public Health in planning to submit their annual report in September and they have asked for any examples from other health departments. I am attaching our most recent Annual Report, submitted in March 2018 and will be uploading it to GovSpace for your use. I want to encourage you all to do the same if you are willing to share.
