[Libs-Or] Public libraries that offer institutional library cards?

North Douglas Library District does this for all in-district students.
Student cards are created at any time during the school year and are tied
to either the teacher or an administrator for contact purposes only. This
allows all children in the district to have a card without parent or
guardian signature. Student cards are limited to two checkouts. We do have
the ability to override for students to check out more. In the nearly seven
years since we offered this, we have lost three books. If a student is
missing a book their card will still check out one book at a time. If they
don't return the book by the following September they start over.
