[Self-Management_OR] Promote Survey to Help Improve Diabetes Management And Prevention in Oregon

Hello Oregon Self-Management Network,


Comagine Health recently launched a survey to better understand top health concerns and how Oregonians feel about diabetes and its impact on quality of life. Many of you have already committed as promotion partners and we received a request to share with the Oregon Self-Management Network. As a valued partner of Comagine Health, we are asking you to help promote this survey to help us reach as many community members as we can.


The results of this survey will be used to guide program adaptations to increase engagement in the National Diabetes Prevention Program (National DPP) and Diabetes Self-management Education and Support (DSMES) services.



To promote the survey, we have created and attached resources to this email.

  1.  CINO social content: Social media graphics in English and Spanish. You will just need to choose your graphic and attach the corresponding caption to them when you post/ attach to emails.
  2.  Newsletter article: Newsletter blurb for those intending on including this in their upcoming and future newsletters.
  3.  Email Script: Email blurb for those able to distribute electronically via listservs or other electronic modalities.
  4.  Press Release: Customizable press releases for CINO partners to share with their press networks.



You will only need to share promotional materials between June 15-July 31. The survey will close on July 31, 2024.
